![]() We recognize the need for a definite understanding between the patient and his/her doctor regarding financial arrangements for medical and surgical care. For those patients receiving financial benefits from insurance or governmental agencies, we are required by law to collect the appropriate co-pay and deductibles specific to your arrangement and the agency involved. The responsibility for payment of these fees is the obligation of the patient. Because of the high cost of statements and billing, we request payment at the time of service, unless otherwise included as part of your surgical or fracture charge. If you do not have medical insurance, an itemized statement will be provided and payment is expected at the time of service. For those of our patients that are covered under health insurance or other governmental programs, we are required by law to collect any co-payments, coinsurance or deductibles that are specific to the benefits that you receive. It is your responsibility as the patient that is receiving services to meet the financial terms outlined under your benefit plan. Because of the high costs associated with printing of statements and mailing, we ask that our patients know the terms of their insurance coverage and pay for any co-payments, coinsurance and/or deductibles at the time of service.
